Welcome to the Parent Profile Registration

This session will cover the following items that must be completed.  There are items listed below that will require additional actions and the steps for completion are included in this registration.

• USA Hockey Registration for your Player, which includes:
  • USA Hockey Concussion Information and Acknowledgement 
    (if not already registered for the 2020-2021 season)
• Parent/Guardian Contact Information
• *DeMatha - Athletic Forms -
  • Handbook for Parents of Athletes Form
  • Permission Form and Release
• *DeMatha - Health Forms/ATS
  • (Four (4) physical health forms to be uploaded)
• *Initial Player Registration Fee Payment- online.  Offline option is also available.

* Additional actions required to complete registration for your players eligibility.

• Emergency Form
• Safe Sports
• Weight Room Waiver
• Handbook for Parents of Athletes Form
• Permission Form and Release

Each Player must register first with USA Hockey and then return to this Registration Session to signup for the 2020-2021  Season.

• Step 1:  CLICK HERE to go to USA Hockey Registration and register your player(s). Be sure to PRINT and SAVE their confirmation code(s). Remember to register EACH of your players with USA Hockey. Each player must have their own unique USA Hockey confirmation number.
